I
will develop a program with my faculty advisor. I will meet regularly
with my faculty advisor.
I
will make satisfactory progress in my research as determined by
my faculty advisor.
I
will maintain the equivalent to a full-time course of study as
determined by my faculty advisor.
I
will maintain a 3.0 G.P.A. Subject to the discretion of my advisor:
If
I fall below a 3.0 G.P.A. for one quarter, I understand that I
will be placed on probation.
If I fall below a 3.0 G.P.A. for two consecutive quarters I understand
that I will be ineligible to continue in the program until I raise
my G.P.A.
I
will attend CEA-CREST seminars (mandatory), and other meetings/events
when required.
I
will provide the Project Coordinator with information and materials
regarding achievements and activities, and current contact information,
while I am a participant of the program.
In
order to receive a full stipend at the prevailing rate, I understand
that I may not exceed more than an average of 10 hours per week
throughout the year of TA-ships or other employment. Employment
exceeding this time-base may result in a reduction or loss of
the CEA-CREST fellowship, if my faculty advisor and the program
director determine that it is hindering my progress towards a
degree. On a quarterly basis, I will report this information to
the CEA-CREST Project Coordinator.
If
I withdraw from the program, I will inform the CEA-CREST Program
Director of my decision in writing.
If
I withdraw or am dismissed from the program for any reason, including
unsatisfactory progress, I understand that as of the date of notification,
I will be entitled to either two weeks or a full month's stipend
at the discretion of my faculty advisor and the Program Director.

on need and availability of funds, I may be eligible to receive
funds to cover the cost of nonresident fees, up to 10 units per

In
accepting the CEA-CREST award and in accordance with Federal and
State regulations, I understand that my financial aid need base
will be met and I will no longer be eligible for financial aid,
including loans. (For assistance in this matter students are urged
to meet with James Howard, Advisor, Center for Student Financial
Aid).
I
understand that if I do not fulfill each item listed above, I
may become ineligible to continue in the CEA-CREST program.
